The background is underexposed.
Bump up the darkness and fill light in LR.

Not sure why you want the background to be brighter. It'll make the main subject wash out IMHO. It's all bokeh there anyway.

Noise Reduction - Luminance & Color.
After applying the noise reduction, sharpening is needed to compensate the loss of details.

Not really, just adjust linearly and monitor the result.
The photo will be blurry if you boost the luminance to 100.
Play around with it and you will know what I mean.

lol there's no need to discard noisy photos.
Adobe Camera Raw 6 has even better noise reduction.
I'm getting impatient to grab CS5.
