I assume you paid for the stuff you bought via PayPal, correct ? But you are not transferring money inside your PayPal account back to Malaysian bank account, correct ? Then you can ignore all the fees listed in PayPal website. It has absolutely nothing to do with you.
It is shown as a hold amount because it has not been posted into your account yet. It takes a few days for the payment to be posted into you account, just like credit card.

won't be able to find out the exact exchange rate used by VISA International. The amount shown (RM157.50) is the rate in RM after they converted the amount charged in USD into MYR and also after factoring in the 1.25% admin fee.
