I think you're dropping your hips too low when you grasp the bar and start pulling. It's almost as if you're starting from a squat.

I got tricep tendonitis about 3 months ago from benching. Did some homework and discovered my form sucked.
Lifting a little less now but benching isn't exctly bread and butter to me anymore.
