Download the form from EPF website and follow the instruction in the forms will do. No more no less.

Just follow the instruction?
Anyway, you can head towards KWSP branch and they have a special counter to check on your forms and documents before you submit. They can advice you better. |

Nowadays, EPF staff is very helpful. You just consult them if you got any problem. Also, sometimes they got both in Housing expo which can help to answer your question. Try go to Modern Living Expo this weekend at MVCC and see whether EPF got booth there or not.

Go to kwsp.gov.my and get the form. The form itself has detailed instructions for you.
BTW, I'm submitting my application this Friday |

Usually when you do the S&P and Loan Agreement, the lawyer firm would have gave you a copy which is chopped.
You'll still need to bring the original for them to sight and verify at KWSP after you have photostat the docs. |

Guys..
For withdrawal from both husband and wife accounts, do we need to submit 2 application forms (1 form for husband's epf account, another form is to withdraw from wife's EPF account). Is it like that? If the are 2 forms, I wonder how does kwsp check the total withdrawal is not exceeding the limit, which is (difference in loan-deposit + 10% of house value). |

I believe your friend case is withdraw EPF to reduce loan, which required the bank statement and need to certified true copy by bank. Bank will do the photocopy for you, thats why bank charge so expensive. I recalled is around RM1 per page when last time I asked to certified true copy of my loan offer letter, end up I don't CTC. aiyooo... no fixed rules lah like tat!!!
