How to fill in the form to get $ from EPF? Anyone with a guide?

Download the form from EPF website and follow the instruction in the forms will do. No more no less.

Unless you don't understand BM then only need guide.

Anyway, you can head towards KWSP branch and they have a special counter to check on your forms and documents before you submit. They can advice you better. wink.gif

Nowadays, EPF staff is very helpful. You just consult them if you got any problem. Also, sometimes they got both in Housing expo which can help to answer your question. Try go to Modern Living Expo this weekend at MVCC and see whether EPF got booth there or not.

Go to kwsp.gov.my and get the form. The form itself has detailed instructions for you.

BTW, I'm submitting my application this Friday smile.gif

Just get the form from the epf counter,u can fill them up there and submit the same day. And this is what is require from me n my wife last week when we went to the epf to apply for withdrawal to buy our 1st house - a completed double storey house with cf.

-2 photocopy of ctc(certified true copy) of cf by developer office or lawyer (this is never stated in the form,but those *ss officer insist us to get them otherwise no withdrawal allow?
-2 sets of photocopy of S&P
-2 sets of photocopy of married cert
-2 sets of letter of offer from bank
-2 copies of I/C
-2 copies of bank statement whether its current or savings account
-Remember to bring your epf statement too, they will be asking you for the epf account number becos some of them is like maybe lazy to check or what i am not really entirely sure.

those photocopy docs like s&p, letter of offer etc. need to be "certified true copy" or can just photocopy on our own?
i think if need bank to certify true copy bank are charging some fees.

Usually when you do the S&P and Loan Agreement, the lawyer firm would have gave you a copy which is chopped.

You'll still need to bring the original for them to sight and verify at KWSP after you have photostat the docs.

my friend last time dunno why, EPF need certified copy of docs of the original.... at the end got to pay 100+ to the bank to get those copies, the bank charged per docs.
aiyooo... no fixed rules lah like tat!!!

in my case only the cf of the house is certified true copy, the rest just normal photostate copy. The epf officer just compare the original s&p against the photocopy of the s&p without ctc, the rest she didn't even check,not sure why. After that she said my cheque will be ready in 2 weeks time.

On tat particular day itself I have to go thru and back from Epf Hq to Puchong IOI sales office just to get the photostate copy of cf and a ctc copy, apparently one officer said without ctc by lawyer she won't approve my withdrawal. She instruct me to come back the same day for submission.

Then off I go to IOI sales office. When i was in IOI sales office,their staff told me they can only provide me with the cf ctc by their sales manager. Then I went back to epf again to submit all the documents. This time is another epf officer. She accepted the epf ctc by IOI sales office marketing manager.

Seem like these epf officer each one is like having different interpretion of the rules and document require. My lesson from here is that if first officer say not enough documents, you might wish to press the queing number and try another epf officer,she might accept them. They are very inconsistent when come to rules,just donno which to follow. Just give me big headache for the run around. rclxub.gif

Wah, so complicated! sad.gif I'll be going to my lawyer tomorrow to ask for documents. I'll make sure he certifies them all! Hmm, hopefully there won't be so much trouble sad.gif I'm doing it in Sunway Tunas, Penang. Anyone did it there before?

Guys..
For withdrawal from both husband and wife accounts, do we need to submit 2 application forms (1 form for husband's epf account, another form is to withdraw from wife's EPF account). Is it like that?

If the are 2 forms, I wonder how does kwsp check the total withdrawal is not exceeding the limit, which is (difference in loan-deposit + 10% of house value).

Ya you will need 2 copy,one for husband and one for the wife. Both will be handle by the same officer for submission. They will assess both account at the same time and they will tell you the max limit both of you can apply for. As for the actual amount of maximum amount for withdrawal,there is a calculator in the kwsp site that you can check.

I believe your friend case is withdraw EPF to reduce loan, which required the bank statement and need to certified true copy by bank. Bank will do the photocopy for you, thats why bank charge so expensive. I recalled is around RM1 per page when last time I asked to certified true copy of my loan offer letter, end up I don't CTC.

Maybe I am the lucky one. We just photocopy all document stated in the EPF withdrawal form and bring the original document, bank book as well. No need certified true copy unless you dont have original. First went to counter to semak document, then went to another counter to submit the application. Get my money within 2 weeks.
