i have done it before a few months ago, if i can remember correctly, you need your snp and IC as well as money for the deposit.
For tenaga, you need to go to the nearest Kedai Tenaga, whereas for Syabas, you need go to their HQ, for people in the KL / PJ, its actually opposite Telekom building in PJ. what is frustrating is, i had to go 2 times to the Syabas office, 1st time is to fill up the form to transfer name, 2nd time is to pay the deposit. They wont allow to do it all in 1 visit as they need to take the meter reading first.
