I think your cat is just showing you that he is mad at you for leaving him for a week 
Cats are funny creatures.  They will show us that they are mad at us by doing strange things.
When I got new members in the family (another cat) my grumpy old man Benji did not want to poo poo in his litter box.
Like your cat, he will poo poo anywhere in the house.
After a while he became his old self again.
For now, you need to shower your cat with lots of TLC.
Don't quarantine him in the cage as it will upset him more.

yea should be some sort like fungal and skin infection. if it is happened last 2 month, and it is ringworm = it should be botak right now...

all parasite, fungal, what so ever bad bugs ehehe love the tail part, neck, face, paws.

im pretty sure u should bring to other vet (not the one u went to last 2 months), just to make sure what is happening to your cat skin...

things to check:
- do human in the house feel itchy?
- do other cats get the same thing as well?
- how do your cat acting? because cat died if too much parasite/fungal on its body.. especially kitten.

I suspect its because your cat has ear mites and the 'blood clot' around the body and neck is from excessive scratching sampai kena luka.  That was what happend to my cat too.  Because of the ear mite in the ears, your cat feel so itchy that he/she tend to scratch near the neck and head thus luka from the nails.

Check your cats ears if ada banyak tahi telinga or not.  How to check? If your cats ears is very dirty with tahi telinga (black/dark colour) then your cats has earmites.  If you clean your cats ears today and tomorrow dirty again then its ear mites.  Go and get a bottle of Cranker Eardrops.  After cleaning the cats ears, put a few drops of Cranker ear drops until you stop seeing the tahi telinga.

About the bulu dropping, it could be some kind of skin infection.  Do get a bottle of malaseb and bathe your cat in it.  I find that Malased is the wonder shampoo for any kind of skin problems.
