If you're planning to buy the number via the counter, just fill the MV10A form. The MV10 form is only for tendering purposes. Also don't forget to provide the photocopy of your IC.
BTW, for numbers out of range of the running numbers, you will be charged RM310 (RM10 for the process fee + RM300 for the number itself). Numbers within the same range will costs you RM210.
Such example would be like this,
Running numbers is 4123, so if you're planning to buy 4556 which is within the 4000-4999 range, you'll be charged RM210. But if you're planning to buy 5012, you'll be charged RM310 for it.
