As far as I know, GPU-Z should extract things correctly for nVidia cards. I have not tried on an ATI so no ideas. Maybe a better explanation and example of what might be missing should be provided

If you know which point to monitor, maybe using a multi meter would be more accurate. You can try google up for "GTX 260 voltage mod" and most of the time there are guides/info on which point to measure voltage.

I've tried using Nibitor to edit my card's BIOS but there isn't an option to select higher vGPU

Something has to be done with the cooling before we go for higher voltage. More voltage but terribly hot won't get it anywhere better.
