I think u should contact with AKPK as soon as possible, they are really willing to help u. First of all, pls register as a member urself online.
For my case, I registered online at the AKPK website, fill in the nessecary details required by akpk, next I make an appointment with the date and time i choose.
I went to AKPK at the date I make appointment, the AKPK officer meet me, and check the documents i bring, remember to print ur latest EPF statement at EPF building before u go to AKPK. Aftertat, the AKPK officer discuss with me the minimum amount I should pay to each bank with the DMP program (AKPK), with the minimum interest rate. I agreed all and the officer start to process my case and ask me to make payment as soon as posible or before 7th of next month.
I received the email from AKPK which told me that I have been successfully enrolled into DMP programme 3 hours later at the same day.
