I want to set up an iTunes Account; is there any difference between setting up a US account and a Malaysian account? Which one is better?

hmmm, i have a problem with apple online store too... Last Thursday i ordered my ipod touch... and to this day, it still have not been shipped yet, although in the online store it said shipping wud be in 24 hours... Is this normal or am i just too anxious?
