I think Suzaku did it. He still believe that Lelouch's memory are not back yet, and he figures that if Shirley knew it would harm Lelouch's reputation.

1 - Rollo will most likely be sent on a suicide mission
2 - We still dunno yet if Karen will join after being given refrain, but that is low from Suzaku if he really does it.
3 - Expect more emo from lelouch if he knows what Suzaku is going to do

No. It's a drug that lets the user experience his/her most valuable moment in life all over again. Karen hated it because her mom (real, 11 one) took it when she was depressed and was thrown into jail for that. That's why she was mad when Lelouch wanted to take it.
