as I understand, they require;
1. IC (name as per S&P)
2. Cukai Pintu (latest bill paid)
3. Grant Title (If any) you may check with your lawyer
4. S&P (Most important)
5. Photo (front and back)

you may call to local Majlis Perbandaran to clarified "this is what I do previously" and you may seek for their advice on recommended architect to help you draw floor plan and they also able to help you to do submission "usually is allow them to do submission"

or you may visit to http://portal.lam.gov.my to look for architect.
