1.There is no fixed rules. Avoid distractions in background. There is loop hole where you can actually chop the model but giving a viewer a feel that you did it on purpose.

2. Blend the model with enviroment is very important. Do not find background that doesnt match at all with the model's outfit. Bokeh or not is up depends on the photographer's pref.
Wide angle shots probably less bokeh. Telephoto normally brings out amazing bokeh.

3. Normally just resized and watermark.

4. Fill-in-flash not always can help in this situation (highly exposed environment) . The only solution would be the master mode (aka manual) Manual metering. Remember this. your camera will not know what exactly you want.It only know how to expose the area you focus, resulted even more blown highlight..
In this case you need to raise up your shutter speed to enable less light exposure captured.
