When you hand in the forms, just sign it in front of your remisier or broker lah. Some remisiers don't really care one, as in just sign it and pass it back to them. Hi, maybe you want to double confirm your CDS account type if you opened via Maybank. I asked the personnel, and she said all nominee accounts only, cannot apply IPO and cannot buy maybank stocks.
