Since you phone it 1.1.1 it quite easy... Just restore the phone using this file

Download it.. In Itunes press Shift + Restore to select downloaded file.

To unlock see

http://iphone.unlock.no/

Follow -> Bypass activation and prepare phone for software installation

and

Unlock the SIM-lock (this one i think no needed since you already unlock)
