Your Punkbuster might not have been installed or updated properly. Use pbsetup to update it manually.

Let me ask you a question, are you using either AthlonXP or Sempron Processor?

You see, this game has a sound file(mssmp3.asi) that does not like any 32bit AMD processor so the game will not start, will crash to desktop and then ask you to try safe mode.

I used to have this problem in the demo but I disabled it by renaming the extension to .bak and the only drawback I had was there was no ambient music.

Recently, there has been a fix found by someone in the community, it is the same file that has been fixed to work on 32bit AMD CPUs. Download LINK. Now the ambient music will work.

What you need to do is go to C:\Program Files\Activision\Call of Duty 4 - Modern Warfare\miles folder and located the original mssmp3.asi file. Then, rename it to mssmp3.bak and extract the file from the downloaded .rar file and paste in the same folder.

Most servers use punkbuster so it is better if you install it. If not, you'll be extremely limited on which server you can play on.
