The supporting columns looked too thin and probably underspec.
Imagine having a heavy slab supported by 2 thin columns. By right it should be thicker or added more columns for support. The house was top heavy to begin with. |

These type of illegal extensions always asking for trouble. Pretty sure no architect or engineering plans.
