Hi anyone have experience of prenatal card lost and not able to do JPN registration for newborn baby?

Hospital nurse took it and lost it.

Please share with me if you have any experience on this.

Private or government hospital?

I once thought I lost mine (private hospital) midway during my wife's pregnancy, they just issued a new one and wrote back all the dates (since they have the record anyways). I later on found the card.
