try uninstall the DA driver.. completely, registry key ..... clean.....try install back.. , if cannot .. i think is windows or ur uSB port problem (hardware)..
Recommended bring it to RMA, can get a NEW MOUSE, If u lucky ....

i experince it b4.. is my windows problem.. some file crashing with the Razer driver...

Regarding that Razer Barracuda HP-1 Gaming Headphone/Mic , got mnay case broken while at packaging .. coz my frenz have been victim b4, the broken part is "Left: side whole spekear drop .lol ...luckyly ban leong exchnaged A new one for he.. (RMA about 2 or 3 week), so next time purchased anything try check first b4 left that store. =P
