i've used those machines before, there should be a safety pin to be slotted onto safest lowest position. i guess this was overlooked by the instructor

Bar alone is usually 10-15kg. The weight looks like 10kg plates at 4-5 pieces each side. So that's  about 90-100kg +- 5-10kg.

Problem here is that she's sitting down so it's almost 100% upper body only and normal people aren't that strong.

2nd, it wasnt a safe environment, if she was going to squat the weight, there shouldnt be the bench there, without the bench she prolly would have survived.

3rd, the impact naturally caused her lungs to pump out all the air, and because of the weight she couldnt take in air, hence the "sudden obstruction of the respiratory tract".

Bad coach, bad practises, bad choice.

Yeah, since you mentioned it, the bench not supposed to be there if she's doing squats
