Thanks to this thread I decided to drop by the Dewan Sivik MBPJ centre this afternoon to try my luck
There wasn't much of a crowd & it took around 30+ mins to get everything sorted.
They definitely have Pfizer vaccines in stock.. from a notice they posted at the entrance they might also carry Sinovac & AstraZeneca shots..
Brought proof of address (some bills/statements) but no one asked for any additional verification other than the MySejahtera app so I guess it's not a worry if you forget to bring any along.
I think foreigners are eligible too as I spotted some people with foreign passports during registration.

Definitely recommend this centre if you'd like a Pfizer shot & don't want a long wait..

Don't come to Dewan Sivik MBPJ today. Lots of people and foreigners.

The foreigners have a hard time due to the forms to fill and slow internet.

They will ask to access FB scan function or Google Lens scan a QR code that will lead you to fill a digital form from Google Docs on the tent before the steps.

Then when you enter the lobby, you need to fill in a handwritten consent form that you will be carrying throughout the whole process. All pretty smooth after that. Do make sure you have internet access as the whole process, they will want to see a refresh page of MySejahtera app.

If you cancel an appointment, make sure it is cleared off before you go as they will deny you if the Cancel Appointment is still there.

No parking inside so park outside at the designated free parking.

And FYI, I asked and it was a strict NO to butt injection. Only arm injection
