All info are based on previous employment income and deductions data in Borang BE or EA form.

If is your 1st job, insert nil.

Provide below figures for the current year only in which you're submitting this form (e.g. figures for 2021 only if submitting this year).
C1 - total gross pay received before any deductions including salary, allowance, bonuses etc.
C3 - total EPF deductions/contributions
C5 - total monthly tax deductions (MTD)

Refer to all the payslips you have received to date for the current year. You need to sum up manually yourself.

EA Form for this year will only be released next year. I don't think you wanna wait. By the time you get your EA Form next year, you would need to fill up the current figures for 2022 instead in the TP3 Form, which your EA Form would not have as it is for 2021.

Meaning from January 2021 till June 2021 right. Manually calculate.

Yes. If you have already received your July payslip, then can include into calculation also. If not, leaving July's figures out wouldn't matter too as TP3 is used by the company to roughly estimate how much tax to deduct monthly in your new role after considering how much tax you have already paid for the current year.

Any excess tax paid still claimable via the usual income tax submission process.

So my previous salary no need pay income tax.. So just leave C5 empty? or put C1 + C3 total?

I think so. Just leave C5 blank or put 0. It wouldn't make sense to combine C1+C3.
