I think you can get it from the place where you buy your phone/gadget ie. Maxis, Apple, Samsung etc.

It's like retailer-affiliated privilege only when you buy from the retailer - maxis, digi, celcom, - and only valid if coverage right at purchase or within first few days from purchase.

i vaguely remember that hotlink have this insurance which you can buy from within its app.
