Help how to remove this black line, and double logout,restart, shutdown

here. i got this problem since last week. suddenly come out. see the black line around folder and text (pic 3 & 4). logout, shutdown and restart also become double (pic 2)

System Preferences > Universal Access > "Seeing" Tab > Make sure VoiceOver is Off....
