It is a vintage-style elevator you see in the movies. To get in and out, there is a steel grate and a door you have to manually pull/push open instead of an automatic door.
The two children got out 1st were being playful, trapped the 3rd kid, who was closing the grate door, by pushing the outer swing door back against him and forced the kid to be sandwiched between the doors.

Unfortunately, the kid wasn't strong enough to open the swing door to escape as the elevator was pulled upwards. The boy was crushed, compressed then squeezed through between the floors.

Was the kid unable to push open the door out to escape or was he playing hide & seek between the doors??
