You have to approach the COA. His signature and rubber stamp are required in the the letter. The applicant also need to acknowledge in his Register Book for record purpose.

I think you need to draft the letter and if there is no ambiguity, he will sign during 1st visit, otherwise, you need to do correction and go again. Is just a simple letter stating what you submitted is correct. If COA provides writing services, then his charges will be higher, otherwise is just 20 to 30 rm in kl.

You can google for COA around your area.
