In Malaysia uually three parties involved.
- medical personnel (usually doctor)
- police
- next of kin

Possible scenarios
Die in hospital and no foul play suspected: hospital will release the body and documents to next of kin's. Next of kins can take d body themselves or assign undertaker. Police not involved

Die in hospital and foul play suspected (murder, accident, negligence, etc etc): hospital or next of kin makes police report. Police will decide further action.

Die at home: usually call hospital. Ambulance will come to certify death, or resuscitate if still alive. Must report police and let police rule out foul play. Police will decide what to do next.

Religious activities/undertaker only involve after police releases the body.
