Actually I'm doing a survey for my Statistic's assignment.
Basically I need to collect 100+/- surveys, and all I had in my mind was LYN forumers.
So those who're going to help me fill the survey form I'm very glad.

some comment on your survey though

1. No name are required(confidential data)
2. You did not state either choose 1 only or multiple answer can be choose.
3. If you just intend to get the output of the data only, there are few free online survey site which you can use for this purpose. They can make your task much easier.

1. You can write your nickname, no need full name, its for lecturer viewing purposes.
2. Normal people will only choose 1 answer only lo in this situation, I think. So that's why I didn't state it out.
3. *I don't know how to answer eh*

Perhaps u'll like to include a range of some sort for people to choose from, especially for income level. I've no idea what kind of survey you're doing but basically, it doesn't seem like it's gonna be able to help much with your research paper.. Anyhow, good luck!

If it's just for an example of probability, u should be able to get sufficient numbers from your classmates - why the need to do such an exercise? Also, can just get ur friends to help. Weird... *shrugs*
