why not ask your relative to go to a normal clinic to confirm whether is it necessary to meet a Ear, Nose and throat specialist. If it is necessary the doctor will recommend you to the a specialist. The doctor will write a letter stating your relatives condition to the specialist and pass it to your relative. Your relative should bring the letter when your relative meet with the specialist.
