For the first image - unscrew it using pipe wrench. Remove old plumbing white tape, clean it up, then wrap again using new plumbing white tape. Screw back the cover and try do the flow test.

One of the best advice ever, originally there's no plumber tape used for this clear out cover and turning it can be easily done. Gave it with few good rounds of plumber tape and screwing the cover back up took quite an effort after that, felt its air tight now.Can google image for those tool if you not sure.
Fingers crossed and monitor for next 24 hours, if its still dry its all good. 👍👍
Note: Thought of hvg nasty stuff dropping out after opening the cover but its surprising clean inside.
