For myself I'll just keep it simple - rebase everything back to MYR (retirement currency) and rebalance twice a year
I'd say it's negligible for popular index funds. I've never had problem getting orders filled by bidding +1 cent for IWDA (typically has 3 cents wide spread)

when you guys buy ETF, do you buy 1 unit or can you buy a fixed amount currency of ETF?

say, 500 EUR. Then you'll receive 1.xxxxx unit ETF?? How do you sell then?

What's so confusing?
If you buy US stocks/US domiciled ETF, get ready to pay 30% tax on dividend.
If you buy ireland domiciled ETF 15% tax.
If you buy UK/HK stocks is tax free.
if you buy china company listed on HK, you pay 10% tax.

That's just emerging market. To start off it's probably cheaper to just buy 1 ticker VWRA which is roughly 90:10 developed:emerging and not worry about rebalancing

Buy as your funds permit lah imho. Hoot/dca one a year 18k not necessarily effective if bull run... if bear bear... then different story
