Just for info.
Those who registering newborn at JPJ Putrajaya will be given ADAM50 form along with the Mykid and Birth Cert. All you have to do it, fill it up, attach with a copy of child's birth cert and your IC. Folks from ASNB will collect the form from JPN and deposit RM 200 into the ADAM50 account. |
