answer to 1 & 3 :-
- system storage will be used for [Game Update + DLC], [Save Data] and [Screenshot/Video Capture]
- physical games will usually have updates and some are mandatory to download to even run the game (eg. some bigger name 3rd party titles)
- if the game does not have any update and/or DLC then system storage will only be for Save Data and Screenshot (if you capture some when in game)
- the storage needed for updates/DLC depends on the game .. anywhere from <100MB to 16GB. on average for the titles most ppl will get eg. Nintendo main titles ... it's about 1-3GB.
Note: the game cart is read-only memory.
answer to 2 :-
- to be exact the PS4 monthly games should be deemed "rentals" since you need to pay for a PS+ subscription (and have one active) in order to access those games. once your subscription expires you wont be able to load the game even if it is installed.
- the Nintendo Online service is only going to see release in another week or two (2nd half of Sep 2018). we will only have actual info about such offer (if they are indeed offering)
- the only thing available free to download and play on eShop is Free-To-Play games eg. Paladins, Fortnite, etc. and of course game demos
