1 - Nasi lemak at Restoran Zaman is quite good. Open for breakfast.
2 - Simple nasi lemak opposite of Megamall is good too. Open in evening till midnight.
3 - Nasi lemak stall in Kampung Tengah is quite tasty. Open for breakfast
One downside about nasi lemak in Kuantan is they don't serve many side dished like in KL.
The only nasi lemak I had in Kuantan 15 years (or more) was at food court. I cant remember the name but Im sure most would know. RM10 (or so back then) for one whole leg (or just leg), one sotong, telur and side dish I think. My friends keep recomendding saying its worth but RM10 back then was quite a lot to pay for nasi lemak.
