I got bad NF-A12x15 fan right out of the box from purchasing NHL12S. Should I contact directly to Noctua to request RMA?

That's my initial thought exactly, but upon closer inspection I can confirm the sound come from the center of the fan which most probably the bearings. If I manually spins the fan, I can easily feel rough vibrations coming from certain quadrant at the center.

Good news, Noctua is sending me a new replacement fan. rclxms.gif
