Your Itinerary cover too many places which I did not recommended. You going for Vacation or going for Marathon Race?
Japan Center + Japan West already.
Your Kanazawa + Ise Shima are Center part, should be Nagoya as Based. Shall keep it as next time trip.
Because of Japan center & west so you need alot of passes.

Since you coming In & Out at Kansai which is Japan West. You shall focus on Japan West. take out Kanazawa for Himeji + Okayama + Hiroshima. take out Ise Shima for Amanohasidate
Focus on West side more then you just need KTP + JR Kansai - Hiroshima Area Pass

Because when we talk about train in Tokyo, Osaka , Kyoto (Top 3 mega cities in Japan) everyone (included Japanese) automatic refer to Private train.
Bigger the cities, Govt (JR) become less dominance.

Keisei are Tokyo
Kintetsu are Nagoya
Nankai are Osaka
Keihan are Kyoto & Kobe

JR don't have direct train from Kansai Airport to Namba. JR Train will stop at Shinmamiya Station, then you have to take another JR line called JR Yamatoji Line to JR Namba Station.

From Kansai Airport, fastest JR train are JR ''Haruka'' Limited Express only service until 2200
2200 - 2343 fastest JR train are Rapid Service Train [Similar to our KTM commuter]

You will landed Itami only you fly within Japan major city.

Any international flight won't landed at Itami

KIX > Haneda > KUL
The tricky part was Haneda airport because ANA will park the aircraft at Haneda Terminal 2. Follow everyone like normal and collect your luggage. Once collect Lugage, Heading to Free Transit Bus to Haneda International Terminal. Once you reach International Terminal, as usual, check in your luggage.....go through immigration....then wait at boarding Gate.

Don't used those international car rental brand such as Avis , Hertz...etc etc
Try Japan car rental such as Nippon rent a car / Toyota car rental.

Each city have different taxes on car rental policy + each state have different price due to season.
