[WTA] NexG prepaid, registration help needed

today I went to am bank branch to buy nexG prepaid, upon buying I was asking to fill up a form, after I went back I open the box I found there are no any registeration form for me to fill, how do I register?
if the form I fill up at bank is the registration form then, how do I know my card is ready for use? |

wht is the number to call?
