btw ringgitplus very slow, 2 days still no email form

i noticed on the cc application form got 'Source Code'.
last tuesday i apply thru ringgitplus, since their email so late, i download form from pb website first.
but today finally got email from ringgitplus.
i must use their provided form right since it has different source code?

if apply thru RP, can still join campaign 2 or not?
i go thru t&c twice, still cannot confirm
