Yah, the offer sort of comes at random, but if one keep subscribing to it, it will be there (the offer). Normally the offer comes 24h before the expiry of the previous one.

Sorry, I'm no so sure because my sim doesn't effected by sim validity as mine is on 2 years validity. My sim can have RM0 credit balance for months. No barred whatsoever. 

Sometimes it will appear promotions purchase RM54 for 1 year validity.

i hadnt tried the plan for 30 days. but i myself had subscribed the 1000GB 7 days plan and it extend another 7 days on the day you subscribed even though my account validity expires another 4 days. So to answer your question, yes i assume it will extend your account validity another 30 days.

Just to check. Is it your sim is old sim or mix and match plan sim. One week past, no 1000gb/7 days seems.

Never subs as i am don't use SIM calling that much.

If the credit is below rm5......the 1000GB won't flower.

I believe the right condition for flowering is

Sim active....,in my case i have bought 2 year validity.
Credit must above rm5.....

Then Flower bloom

At that time ...that 1000GB missing from offer because my credit that time fall below rm5.....i wait wait long long..
...then i realise maybe because my credit is below rm5..
Then topup....Boooom
Next day Flower blooming.....

So..i believe credit must be rm5 and above at all time and also sim active lah.
