2. The form states e. Salinan dokumen pengenalan diri Saksi. Who is eligible to be the witness?

the Saksi can be their staff if you do it their OFFICE. Only applicable if the Beneficiary is present.

3. This process can only be done at ASNB offices and branch? We can't do it at agent banks right?
Bank cannot. Certain branch do not have Comm of Oath. so best u call and check.
