PS3 stop production in Japan only. Asia is next.

Europe and US will continue production until at least end of 2018.

1) Turn off PS4 completely.
2) Press and hold power button on console until the second beep.
3) You're in safe mode.

What TV are you using?

Sounds to me it's a HDCP handshake issue.

I wish.
But display port does not work properly on the PS4. An active adapter MIGHT work though, but you still won't get 4k60 since it's max output resolution is only 1080p60.

PS4 Pro though, I have not tried.
