May I ask is the Application Form from branch same as the form from website ?

I can well prepared and just submit when reach bank, no need fill up detail again

not sure about the forms but i did not filled up anything.
she volunteered to fill all the details for me.
just need my ic and company business calling card.
and the supplement card holder ic.
and requested me for payslip.
submitted all those vide email.
superfast. less than 5 minutes for everything i think.

I see, is it good to process if bring photo copy of supplement card holder IC or the person is need to be there?

I applied at KL Sentral branch. No, the officer helped me to fill in.

Yes, around 1 month for AmBank too based on my own experience.

1 month still good to go... assume October get... just nice for year end traveling

They are slow and steady and still using ancient technology, so bear with it.
However, my experience of applying in KL Sentral branch is quite a pleasant one.

Went to Ambank to check about credit card information.

The staff told me that the income requirement for Ambank Platinum & Visa Signature are the same still 60K & 100K

I said, website is different than the form, she insist to follow the form which download from Website

If I were you, I will argue about it. Either get someone to amend your misleading website or accept this application.
