If I recall correctly, there is no form to download. Unless you can please share the link???? Please share information based on actual experience or facts, otherwise it may confuse people.

What I did early last year was email CS with a request for permanent CL increase and 3 months payslips. It was approved in 3 days if I recall correctly.

You may want to call CS to double check if it is still the same process.

I just called CS for you and he clarified the following:

1. There is no form.

2. Just email CS to request permanent credit limit increase and attach "supporting income documents". Best documents to submit are either 3 months payslips, latest EPF statement or last filed tax return. If you don't have those documents, you can submit things like employment contract, offer letter, or promotion letter. The credit limit team will assess and revert to you if more information is required.
