Time to request for bootloader unlock code again, since last time i request didnt receive anything from Xiaomi

I requested to unlock bootloader many weeks ago, until now didnt receive the unlock code
Now when I go to http://www.miui.com/unlock/ I cant retry to apply, it just direct me to download the unlock tool.

What should I do?
