One moment it's FOC, the next it's chargeable.

May I request for a definitive answer for the following scenarios, please?

A) Changing of old existing Micro-SIM into Nano-SIM: Chargeable? YES/NO
B) Replacing a broken/damaged SIM card (regardless of size): Chargeable? YES/NO
C) Porting from a different Network Provider to Maxis (regardless of SIM card size): Chargeable? YES/NO
D) Getting the SIM card replaced (under any of the above scenarios) at a Maxis Authorized 3rd Party Dealer: Chargeable? YES/NO
E) Are 3-in1 SIM cards issued as standard across the board, or is it only upon request? Also, is there a cost/fee involved for such instances?

I went to Maxis Service Center in Sunway Pyramid. There is a SIM card dispenser machine.
You slot in your Mykad.
Key in the mobile number you want to change the SIM card.
Do a thumbscan to verify your Mykad number.
Once verified, a new SIM card will dispense out.
It is a 3 in 1 SIM card.
Regarding the charges, it depends on your Account. Mine was free, no charge.
Within a few minutes your mobile number will be using the new SIM card.
Make sure your contacts are not stored in the old SIM before you go to the machine.

That question can only be answered by Maxis.

Please be informed that there will be no charge for first time SIM Replacement. If more than one time, there will be RM10 charge on the change. However, the charges will be based on your scenario. You may get further advise bu our agents at any nearest Maxis Centers.

Dust in the chip normally the cause of it...
Banks ATM card also like that.. cannot read. Use eraser okay 👍
