A) Changing of old existing Micro-SIM into Nano-SIM: Chargeable? YES/NO
B) Replacing a broken/damaged SIM card (regardless of size): Chargeable? YES/NO
C) Porting from a different Network Provider to Maxis (regardless of SIM card size): Chargeable? YES/NO
D) Getting the SIM card replaced (under any of the above scenarios) at a Maxis Authorized 3rd Party Dealer: Chargeable? YES/NO
E) Are 3-in1 SIM cards issued as standard across the board, or is it only upon request? Also, is there a cost/fee involved for such instances?

I went to Maxis Service Center in Sunway Pyramid. There is a SIM card dispenser machine.
You slot in your Mykad.
Key in the mobile number you want to change the SIM card.
Do a thumbscan to verify your Mykad number.
Once verified, a new SIM card will dispense out.
It is a 3 in 1 SIM card.
Regarding the charges, it depends on your Account. Mine was free, no charge.
Within a few minutes your mobile number will be using the new SIM card.
Make sure your contacts are not stored in the old SIM before you go to the machine.
