As the title indicates, assistance is needed here to fill up the application forms for SIRIM Permit. My English is still ok but facing the forms for the first time, I see many options that seem similar to one another, and certain terms I do not want to confuse myself with. I am applying for a single electronics item for personal use. Do I still need to fill in everything, because I feel like some entries are not relevant.

Consignor --- Seller?
ROC/ROB?
Agent --- Courier?
Permit category?
Declaration and Undertaking? (authorized representative, company stamp?)

etc. etc.

Any help will be greatly appreciated! Thank you.

No, in fact my item hasn't reached Malaysia yet. But sooner or later will need to fill those forms also. I received two forms from an email by SIRIM. I think the forms are still the same from a few years ago. One is CMC form, another is SAC form. Isn't HS about the tariff code instead of permit category?

So we cannot gaodim application if the item isn't held by kastam? I think i need to contact 3 parties now, the seller, customs and courier for the details. Let's see what happens at custom first. My main problem is still the difficulty to go to Shah Alam to submit the documents.

Ok. Will need to ask her if application thru email can be done. Really can't be present at the HQ itself because someone needs to be taken care of at home.

So two days ago the package was finally detained at custom. The funny thing is no one sent me any mail, FedEx didn't call me on that day itself. I needed to call them myself.

I just finished submission of the forms today and an hour later SIRIM sent me this. I'm not sure what that means. Do I need to send my phone for them to test within 14 days? Why though? confused.gif shakehead.gif

Just read from older posts by others that it can be ignored for personal, I guess I'll do that. Also dled a few pdf files to read about it.

No. We don't have to do anything about that. Custom being cleared is the last thing we need to worry about. That extra procedure should be relevant to companies.
