I had the same issue as well. At first I thought my GTX 960 couldn't handle the graphics. Then after extensive googling various forums, its a known issue after the latest patch. Many players of different hardwares are facing this issue as well. So what I did was go to My Document\My Games\ and delete the entire Division game folder.
