Had this problem with my old ps4, but with Titanfall 2. Its could probably be your blu ray disk drive at fault. Does it make a constant whining noise while trying to load? Mine had that, like it was failing to spin the disc properly.

If its that, you need to send the PS4 for repair. But as a short term workaround press down on the disk firmly while inserting it into the PS4. Another way is to stick a solid flat object (a thin ruler) into the disk drive to the top of the disk and pressing it while the system is off/rest mode but that could damage the top of your disc a bit.

Try delete the game and reinstall again.

Where u buy from? Just go service centre and demand new set.
