Hi, my cousin is doing a survey on Heart Failure and he really really needs your help to fill in a survey for his project.

No background information, no course, no institution, no name of supervisor (if don't wanna provide the name of the student at least provide lahh the name of the supervisor), no research topic, what also don't have.
