The reson for failed request is related to your account (personal details) is incorrect. Hence, we suggest that you pay a visit to Maxis Centre to get your details updated correctly and accordingly. At the same time, you may need to send a reply as confirmation to your port out request within its valid period. Otherwise, it will also leads to failed request.
